AWANA Club 2018-​19 Registration Form

Club Begins: September 14, 2018 – Ends: June 7, 2019

• Time: 7:30 p.m. – 9:00 p.m. (every Friday Night)
• Age: 3-12 years old (preschool – 6th grade)
• ­Registration fee: $35.00 per child, Due: $1.00 each meeting
• ­Each child pays for his/her own Awana uniform and handbooks
• We will need parents’ help in AWANA meeting as helper; parents will be asked to sign up for one month during a club year.
